How to Register:
Please READ ALL FORMS carefully and then note that BOTH forms (3 pages)
must be completed, signed and forwarded to register.

REGISTER ONLINE   REGISTER BY MAIL
1. Complete Form 1 - General Information -
do not enter credit card information in this form		   1. Complete Form 1 - General Information  -
2. Click the 'Submit General Info Form'   2. Print the completed General Information Form using the print key / function within your browser
3. Upon receipt of your General Information Form, a Surf School representative will contact you to answer all questions, discuss payment options and Release Form submission information   3. Click on the link for Form 2 - the Release Form
    4. Complete the top portion of page 1 of the Release Form
    4. Print page 1 of the Release Form using the print key / feature within your browser
      5. Fill in the remaining questions at the bottom of Page 1 of the Release form by hand
      6. Click the  'Next Page' button to go to Page 2 of the Release Form
      7. Print Page 2 of the Release Form
      8. Please Complete Page 2 of the Release Form in the presence of a Notary Public and have the completed form stamped by the Notary Public.
      10. Send all three completed and signed forms, along with a check made payable to Lisa Mead and sent to Lou Maresca & Lisa Mead's Central Florida Surf School, 105 River Oaks Rd, Melbourne Beach, FL  32951
  * All payments and deposits are Non-Refundable.  If you cancel, you will receive a Rain Check good for any time in the future,  conditions permitting.
  * We reserve the right to cancel any class due to adverse conditions
